fix: rename Appearance to Theme in settings and setup
fix: apply a theme only after Save; Cancel, Escape, and failed saves no longer leak the selected theme
fix: repaint already-visible transcript lines when the theme changes mid-session
fix: remove hardcoded green/red/yellow so every accent-colored element matches the active theme
fix: order every confirmation as Save then Cancel, and standardize Save/Cancel across theme, provider, model, credential, and integration changes
