Maybe the proceesing instruction <?xml-stylesheet href="build/xsltforms.xsl" type="text/xsl"?> has a typo in your xforms. Some browsers don't need the ? for closing processing instructions, so they will recognize this: <?xml-stylesheet href="build/xsltforms.xsl" type="text/xsl"> without the ? before > as a valid processing instruction. Check it. Also make sure you clear the browser cache after changing the mime types in your server. I'll be helpful if you can send a test case here.
