Changes and extensions to load_dyn(c).
* Added load_dyn_gen/2 which allows the user to make a call via an option list.
* Changed a warning about too many non-canonical terms in load_dync to a syntax error.
* Made the number of non-canonical terms encountered before throwing an error settable via a prolog flag.

The change to curr_sym is to add a syntax_error/2 (there was only a syntax_error/1).

added a new flag for an adjustable error limit for load_dync.

Declared decode_index/2 exported in curr_sym and commented back in an index decl in file_op.H (and commiting consult.xwam that seemed out of sync).

Just a little cleanup that I forgot to do.

Somehow I forgot this one.

Fixed an incomprehensible error message, and added a new predicate to find the module of a term and format the term with the module for errors and debugging.

Forgot to add this earlier.

Fixed table abolishes for table_indexed predicates. I broke this when I added
a hook for not_exists, but there wasn't a test for it in the test suite. SO I added a test also.

Added a test for tabled answer subsumption executed within a module along with a test of abolishing table_indexed predicates.
