Modifications to reflect new names of incremental tabling predicates.

Removed unused tests from table tests (they ahd been moved to incremental_tests)

Changed a misc_error to a permissions_error when trying to invalidate an incomplete table.

Mostly minor changes to incremental tabling section to change names from _update to _immed

Change all incremental calls to lazy (inval) ones.

Somehow this one didn't get removed.

Added test for throwing over nonincremental tables, and moved another incremental tabling test from table_tests to
incremental tests (incremental tests are presently not executed by the MT engine).
