#93 Custom Fields on Activity Form

I am a newbie to xrms and have minimal php knowledge. I have gone through the docs and forum and understand (somewhat) the link between Opportunity Status and Activity Forms.

But can someone give me a step by step process for adding a custom field to an activity form or direct me to where that step-by step process could be found?

I have custom_fields enabled and nothing else in the plug-ins.

I just want to see if this system will work for my needs.

Background - My business does request of information services for doctors. I need to be able to add activities that require information on type of insurance, patient, requester, and several other auditing fields.
