#81 Allow multiple pages with comments in a module

Currently comments can only be placed on one page in a
module.

I would like to be able to attach comments to different
items in one module.

Perhaps specify comments in xoops_version with an index
as it is known from blocks, templates and notifications
and have page/item name pairs and select the pair to
use based on the current page loaded.
