#22 Fake mouse remapping

This patch allows command-line specification of the modifiers to use for fake mouse buttons. The -fakebuttons option and GUI preference still set mouse2=command and mouse3=option, and the new -fakemouse2 and -fakemouse3 allow any combination of modifiers to be used for both buttons.

This patch also fixes the fakebuttons bug when the modifier was released before the mouse button. The code still can fail if fakebuttons and real buttons are used simultaneously.

This patch collides badly with #441939, Modifier remapping.

    This is incorporated into CVS with minor changes:

    - Faking requires matching the mask and having darwinFakeButtons be true. Otherwise this gives command line anomalies such as using -fakebuttons after -fakemouse2 erases the change made with -fakemouse2.

    - The non-functional Preferences hooks aren't included. This will be fixed when the Preferences are overhauled with a proper UI for all recent features that are currently only accessible via the command line.

    - DarwinPressKeycode() is a macro, not a function.
