#66 Need license info in code

We need to have a LICENSE file at the top of the repo. Also, best practices are that every code file should include license information at the top. The license is 3-clause BSD, with minor modifications to satisfy our lawyers.

Jay Billings has scripts to automate injecting the license into the code files, and he knows the right license to use.
