These changes all seem to work here. My HDTV is still OK. Interestingly, removing the preview buffers *doesn't* stop the initial H.264 errors when playing TS files. This might be related to establishing audio/video sync, I suppose...

Those two changes need some testing.

I don't expect anything outside mpeg-ts to fail because of this (other
demuxers still send preview/header as before).

And mpeg-ts uses only few codecs so it should be quite easy to test.
