The problem is well- and long known (see https://sourceforge.net/p/xindy/mailman/message/24050164/). AFAICS the fix is rather straightforward. At least it is rather easy to fix the dataloss (\see[also] entries not printed with hyperref). The missing feature (such \see[also] entries also linked with their reference in the index) can be added later.
I suggest to apply the attached patch to make xindy/hyperref at least basically functional again.
Note that \see[also] entries are not linked either with their target with makeindex/hyperref, so this patch gets both on par.
