#19 ignore-hyphen and ignore-punctuation are garbage

The texindy modules ignore-hyphen and
ignore-punctuation are garbage. Punctuation (including
hyphens) are handled especially by make-rules modules.
There, they are ignored until run 3, where they are
sorted after letters.

We should document that somehow.
