On checking out the v12 trunk in the svn, are the following few empty folders needed?

www/images/rrd
etc/install/include
etc/defaults
build/ports/{gzip,phpmyadmin,unison,iperf3,nano}/files

yes, most folders are needed. Although they are empty in svn doesn't mean they are not needed.
If you enable monitoring, you will notice that /usr/local/www/images/rrd gets populated.
Other folders will be populated during runtime/buildtime as well.
