#14 Instances with invalid image numbers

There are, on occasion, instances with invalid image numbers in their DICOM header. These images have either 0 or no image number at all, which throws off the Xebra-Client.

We've decided it's better to simply fix these images as they're coming into XebraSCP rather than dealing with them later.
