Hello and many thanks for your wonderful x264vfw. The only problem I'm having is when outputting from latest Adobe Premiere CC (AVI/x264vfw). When I output audio (any format) for exactly the same project/video, the result will always be out of sync, the video is behind the audio for about 500-600 ms.
This could well be a Premiere problem but since I only get it with x264vfw I ask in case some codec setting change for example could help.

Try with "Zero Latency" option enabled. Or use direct output to file instead of VFW in "Output mode" and specify file in "Output file" (different from the one you use for output in Premiere). After that you may need to mux video from specified file and audio from Premiere output file.

Many thanks BugMaster, just tried with a small video and both your suggestions work. Now wondering which to use... Zero Latency mentions "at the cost of compression efficiency", this means lower quality output with the same settings?
The File Output mode writes two files simultaneously during encode if I'm not mistaken and that's the only drawback?

Yes. Zero Latency will cost you compression efficiency (i.e. lower quality at the same bitrate) because it will disable use of B-frames and MBTree (and few other options) and will use slice-threading instead of frame-threading.
As for File Output mode its only drawback is that it writes in separate file which would have only video stream (i.e. without audio). But you don't have any quality drawbacks with it.
