#13 multiple documents with same doc_id and active flag

In an existing installation the documents table in the
database appears to have multiple rows with the active
flag for the same document. While these have the same
version number they appear to have the same time
stamp and therefore all have the active flag.

Perhaps this is the result of a database import?
