I see some problems formatting tables in resulting html

Here is a summary of what I get:

<table width="100.00%" border="0" cols="4" rows="3">

<tr><td bgcolor="White" width="22.88%" rowspan="1"
colspan="1">....</td></tr>
<tr><td bgcolor="White" width="77.12%" rowspan="1"
colspan="1">....</td></tr>

<tr><td bgcolor="White" width="22.93%" rowspan="1"
colspan="2">....</td></tr>
<tr><td bgcolor="White" width="77.07%" rowspan="1"
colspan="2">....</td></tr>

<tr><td bgcolor="White" width="22.93%" rowspan="1"
colspan="2">....</td></tr>
<tr><td bgcolor="White" width="77.07%" rowspan="1"
colspan="2">....</td></tr>

</table>

The table is not appearing properly since the first row
has 2 columns with colspan=1, the last 2 rows have 2
columns with colspan="2".
The same table in the original document looks just fine.

A couple of questions:
1. Why the conversion assigns cols="4" rows="3" to the
table. Is it the original format of the Word table.
2. Any idea how it happened that different rows have
different number of columns.
3. Is there any way to fix it?

I did not create the original doc and do not see anything
wrong with it. I wonder if it was a result of merging cells
in word or something like that?
