bugfix: empty source data fields are now correctly handled

Bugfixes:
- URL handling;
- data files with no extension).
- fixed type handling to include types sql.Date, sql.Timestamp and sql.Time
Enhancements:
- select count(*);
- added possibility to specify date format;
- in case of fixed length columns, I added the possibility to specify whether input VARCHAR fields have to be trimmed or not.
