Am not sure but I think MS Photo Editor is biorked in 1.0b. Though HTML-Export looks ok for images. --kevingpo

The original author had abandoned support for MS Photo Editor conversion in favor of HTML export.
We should probably drop support for Photo Editor altogether.
Say, Kevin, what do you think if we drop support for Photo Editor? It seems the author was moving that way. I've never used it anyway.

HTML-Export requires PowerPoint as I tested.
Actually, after reinstalling Office (to fix the W2MW+ parasite toolbar - which still won't go away) I found out that all W2MW+ versions (0.6b, 0.6e, 0.7b, 1.0b) Photo Editor image conversions were good/consistent.
So this bug can be gone.
Damn, can't edit previous comments. Anyway, to solve this users must select:
Paste in MS Photo Editor as New Picture.
I don't know why.
I attached a WinRAR of image attachments.
HE = HTML-Export (image 3 never got converted???)
PE-Bad = Photo Editor without 'Paste in MS Photo Editor as New Picture' ticked
PE-Good = Photo Editor with 'Paste in MS Photo Editor as New Picture' ticked
Apparently, it looks as if HTML-Export needs a bit of work, not Photo Editor.
Just a recap on the 'Image handling' tick boxes (assuming 'Image extraction' always ticked):
1. First-time extraction/conversion - must have at least 'Paste in MS Photo Editor as New Picture' ticked. If you have 'Paste in MS Photo Editor as New Picture' unticked, nothing happens. If you have 'Image upload to wiki' ticked then a black & white, zoomed-in, cropped conversion takes place.
2. Not-first-time extraction/conversion - must have at least 'Update existing image files' & 'Paste in MS Photo Editor as New Picture' ticked.
