I had the same problem with my Orinoco Gold card. Believe it or not, 
the en1 interface appeared after physically jiggling the card!

I believe that if your en1 or en2 interfaces do not show up after a 
proper installation, you should remove the card with the machine on and 
reinsert it. The interface may then appear.

Contrary to the FAQ, once the interface has appeared, and I remove and 
reinsert the card, I get kernel panic and have no choice but to restart 
the machine.
