Create a monospace id and interpret `backtick` escaped
text as monospace.

Monospace is good in technical articles; most books
monospace shell commands and C functions. Some
monospace C++ classes (and possibly objective-c
objects), others monospace AND use small caps. Small
caps are bad for illustrating case-sensitive concepts,
be wary.

Technical articles would be written to monospace things
like the unix commands `ls` and `cp`; and the C
functions `mprotect`() and `strncpy`(). These are
illustrated in the proposed mediawiki notation.
