#47 excise standard I/O

Excise all uses of stdout & stderr
Excise most uses of pout & perr

Acceptable uses are in the log.cxx module, and in other obvious
disaster situtations.

Some libraries (notably Antlr) use stdout/stderr in some places.
We'll need to override classes to get around these where needed.
