#11 Detect unplugging / plugging

There doesn't seem to be any support for detecting if a controller object has appeared or dissapeared, at least not on windows. After initialization getNumberOfDevices doesn't seem to change. I'm assuming the correct usage isn't to recreate the input manager every frame.
