#593 new_entry.php URL error

The current code assumes a date was added to the new_entry URL string and when a user is added, it's always added with & This results in a URL error if there is no date. The proper thing to do is check if the URL string has a ? and if so, it must exist so add the user with an & If the URL string has no ? and is not empty, then add the user with a ? .

Only /includes/menu/index.php needs to be changed
