In addition to the missed installation information in the
documentation , I found several bugs in the scripts:

1. Package UTGEN is not created because lv_package
local variable is not declared in the testpkg procedure.

2. Tables UT_PACKAGE and UT_SUITE are not
dropped during uninstall due to existing foreign keys from
table UT_RECEQ_PKG.

3. Public synonyms UT_REFCURSOR_RESULTS_SEQ
and UTPLSQL_UTIL are not dropped during uninstall and
cause an ORA- error during re-install.

    I have not seen any of these issues when installing/uninstalling the current version (v2.2), so I assume they've been fixed since this report.
