#132 Multiplexor node has wrong name

When creating the multiplexor node for a context node of the type x = referenceOfY(y), it should be named referenceOfY(!y) and not CX#Y. The problem is that if the same context node is used in another MFrag, a different multiplexor node will be created with name CX#Z and this would be an inconsistency, since the semantics of the node should be the same, i.e., referenceOfY(!y). This node should be reused instead of creating another node.

    Fixed by using suggestive names in multiplexors, and by not duplicating such multiplexors when they are present in multiple MFrags.

    The issue of multiplexor nodes always having uniform distribution is still present, though.
