#17 Add survey for locked resource

When adding a survey to a CD-ROM from a Nesstar file, a
"Loading Resource Failed" dialog is displayed if the
Nesstar file is currently open by another application.
The dialog options currently are: "Try Again", "Close"
and "Ignore". There is also an option box to "Remember
my decision".
This should be changed to: "Try Again" and "Cancel" and
the "Remeber my decision" checkbox should be removed.
Note that choosing Ignore or pressing escae at this
time has unpredictable results.
