The XTMBuilder should be able to handle a <scope>
element appearing in the wrong position as a child of
<association>, <baseName> and <occurrence> elements
when DTD validation is disabled.

Added a test for various lax parsing features such as
out-of-order scope and instanceOf elements. The parser is
still probably not bullet-proof against invalid XTM files,
but it should accept a wider range of malformed topic maps.
