so many things to consider. sigh..
I see your point, that the local copy may be altered or vanishes.

Maybe this is leading too far and we should just use your first
proposal:
/*
 Copyright (c) 2000-2004 The TM4J Project. All rights reserved.

 This software is licensed under a modified version of the 
 Apache Software License. The full license text can be found at
 http://tm4j.org/LICENSE.txt 
*/

This is short, simple and non-ambigous.

In the unlikely case that the license will change, we will create a new
stable adress, something like  http://tm4j.org/LICENSE-2.txt and update
all relevant resources.
