Crashes immediately after clicking "Decode"

After enter the input and output file and my TiVo's MAK, this program crashed immediately after clicking "Decode". Here is the usual Windows crash message:

TivoDecode.exe has stopped working

A problem caused the program to stop working correctly. Windows will close the program and notify you if a solution is available.
