[Tiny-cobol-users] Converted indices to use binary (usage comp) variables

Hi,

I have changed David's work a little, to allow use of binary variables as
indices, and introducing a new gen_set function to generate inline code
for doing the index calculations (it's much faster than calling a
C function).   Please revise it at test.code/t16.  Look at the assembly
code generated to understand what I mean :)
