#1582 Wiki: Structures create page error

After creating a structure, I tried applying one page
twice in the structure.

After that, I editied the top page and added "{toc}".

But when this top page rendered, it no longer displayed
the structure navigation table, nor any TOC.

Then when I tried to remove it, I got an error saying
the page does not exist.

And now I can't remove it.

I'm guessing this corrupt page has something to do with
the attempt to applying one page twice in a structure
and then trying to render the TOC.

This seems to produce the expected results when testing with
1.8.3 with the exception of TOC being displayed (I'm not
    sure why).

First question is are you able to reproduce this problem,
c.e. creating new test pages and a new test structure? No
matter how hard we try, it will be useless to try to
    reproduce if you can't yourself.
Then you might also want to test with an up-to-date version.
To precise your problem, make sure to mention the URLs
involved at least when editing the pages to insert "{toc}"
and the URLs where unexpected behavior was observed. You
could also mention whether or not Wiki caching (Admin->Wiki)
was used. I would recommend to first disable it for testing.
If everything's fine then enable it and see if things break.
