WSGI branch: fix CGI error message, some minor cleanup.

Improve SMTP header format in ticket notification (long ticket summaries led to display issues w/ some MUAs):
* max header length can be configured
* email library Headers are always used to encode MIME headers
* new notification test
* ''anonymous'' user is always removed from the recipient list
