#280 Refresh feature doesn't reestablish modal dialog behavior

The new refresh capability of ThinWire does not currently reestablish the modal layer that is in place for a modal Dialog after a refresh/repaint has occurred. So, for example if you have a modal Dialog displayed and then you refresh with the reloadOnRefresh init-param set to false (the new default value), the Dialog will no longer be modal after the refresh/repaint occurs.
