I find using SHIFT+INS to add a note below another a little awkward, this is a common action when making many notes. I feel a more natural workflow would be to use RETURN to place a note below (as per carriage return)and SHIFT+RETURN to create a child note. Having user definable keyboard shortcuts would allow each user to adopt their preferred workflow.
