If
- a staff is magnified by exactly -0.5
- a chord contains the interval of a second, so one notehead of two needs to be offset
- of those two noteheads, exactly one needs a ledger line
- the chord contains any number of other notes (that are not also clustering against the two mentioned notes)
- and the stem points ‘outward’
the necessary offset of one notehead doesn’t take place.
