has caused me to mistakenly close windows and waste a lot of keystrokes
before I remember to alt-tab back to my .ly file. A definite usability
deficit.
BTW, this happens on both my office machine (running XP Pro) and my laptop
(running XP Personal).

I have *never* had a problem as you reported in (2) I use Lilypond on Windows 7 and have tested on XP. I am assuming you are referring to either right-click open with... context menu and/or once you have associated the .ly extension with notepad that when you double-click it opens in said program.
I have used 2.12.2 and pretty much most of the 2.13 builds (I use 2.13 solely now).

I believe my latest update to lilypad fixes issue 1. Nobody apart from Graham has ever seen issue 2. I believe this is now, therefore, fixed.
