#584 XHTML a pointer without content in an automatic index

In certain circumstances index-term gives in XHTML an index entry with a pointer XHTML a element without content.
See more explanations and wrong behaviour in index6_35.html obtained from testingthings.xml. From an older state of the stylesheets I had at hand, I get a better behaviour (the same text then lies): index11625.html.

    hmm. thats because the template called "header", called by the index maker, finds the most recent object with an XML ID. Not the best plan. Fixed pro tem but really that whole index maker is simplistic.

    the next release (7.7.0) should solve this. the fix doesnt seem to have any ill effects
