#516 Documentation of <equiv> is confusing

The definition of element <equiv> originally had only pointers to four sections in chapter 22:

[22.4.4 Element Specifications 22.4.5 Attribute List Specification 22.4.6 Element Classes 22.4.7 Pattern Documentation]

but <equiv> seems to be mentioned only once, parenthetically, in the entire chapter. It's documented more fully in the CO chapter, so I've added the link there as the first pointer, but we need to decide if these original pointers are there because there was an intention to add text and examples to the TD chapter, and it never got done, or whether these original links are just misleading and should be removed.
