The little green maximize (+) button appears to do nothing for TkAqua
windows. Is there a way to make it work, or is this functionality
(Surprised I didn't notice this for so long, but for some reason I
don't often maximize windows on Mac OS X).

No, this doesn't work at present. There is a TODO in TkpWmSetState to
handle the zooming, but that isn't quite enough. You would also have
to register a Window Handler on the Mac OS X windows as they get
created for the zoom event, and then handle that event by storing the
original size and some "large" size, and toggling between them. I
don't remember if there is any way to ask the various geometry managers
"what is the biggest size you would like to be". If there was, that
would be a good "biggest size" setting.
