[TCLCORE] stacked channel docs

I've rewritten the docs related the Tcl_CreateChannel (CrtChannel.3) and
Tcl_StackChannel (ChnlStack.3) to reflect the "new way" of things.  Those
concerned might want to check them out just in case I sound off-my-rocker.
They are in the core-8-3-1-branch, not yet in the HEAD.
