[MACTCL] Update on buttons

Hello all,

Marc Culler has graciously submitted a patch that cleans up the button 
metrics in HITheme on Cocoa; I've tested it via the Tk demo that comes 
with Wish, and everything looks as it should--the same as it did under 
Carbon and nearly indistinguishable from Cocoa. His patch simplfied the 
geometry layout and removed a lot of special-casing I had put in.

Marc also submitted a smaller patch that tweaks the layout of ttk 
notebook tabs so they look consistent with Cocoa notebooks under Yosemite.

Thanks to Marc for his work! I think we can declare the button updates 
complete at this point.
