Enter task with template doesn't show entry form

On one machine (Mac) when I choose enter task with template, I get a form to fill in, which is what I want. Field values that are part of the template are filled in.

On another machine, also a Mac, when I choose enter with template, I get a new task in the task list with pre filled values. But, I never get the entry form. I want the entry form to come up. It is very difficult to fill in within the task list because of infinite horizontal scrolling and the fields are just bad.

So, what am I missing in the labyrinth of settings that will make the entry form appear? It looks the same to me on both machines but it is easy to miss one of the hundreds of inter-related settings.
