trying to run strain gradient model without strain gradient terms activated

changed to reflect change in input file due to un-hardcoding of ghoston/offmap in BridgingScaleManagerT::Solve()

modify C8 back to original value

updating example

added test of velocity gradient calculation for uniaxial extension

adding plasticity example

but not as benchmark

added following capabilities:

1) maximum porosity cutoff (PHI_CR)

2) rate-insensitivity flag (only for D_IMPL_OPT = 4)

3) damage nucleation term (DNUC1-4)

change back to select Sierra_EMMI

adding another example to run just MR plasticity with meshfree (no gradients)
