large arguments to the sinh are causing floating point exceptions on
CPLANT. added check and exit on error if arguments are too large.

clean up specification of output variables

added limiter on the maximum amount of damage the model will accumulate. If
the model solves a state with damage exceeding the maximum, or fails to solve
a state but exits the stress point iterations at a state with damage that
exceeds the maximum, damage is set to the maximum and force, i.e., ncd = 0.

added total Lagrangian solid element with simplified routine for
calculating the residual force

limit the model to elastic response during iteration -1, i.e., the first
residual and solution update for the given load increment. That is attempt
an 'elastic' iteration first.

(1) enable step cut signal
(2) limit max update to damage during local Newton solve to 5%

Minor revision to make use of Alias instead of Set.
