updated for changes to DOFElementT interface and implementation of recovery
from solver failure over the current time increment

corrected for surface normal in +/- y-direction

replaced rescaling of update vector for phi < phi_init with an exception
that's thrown if phi < 0

don't create constraint pairs between upper and lower facet nodes that
are the same, i.e., self-self constraint

(1) corrected initialization of kappa (now 0.0)
(2) changed limiter on max phi

tracked down dimensioning errors, but not converging because
dof numbering is off

still playing with dimensioning

(1) use phi_init from the input parameters to initialize the initial damage
(2) added lower bound limiter on phi during local iteration to solve for
stress and updated state
(3) clean-up

enforce constraints at all integration points again, not just at the
element centroid.

attempting to fix OutOfRange exception; still not working
