[sword-app-tech] namespace error in examples of spec v1.2

Hi,

the sword spec version 1.2 at

http://www.ukoln.ac.uk/repositories/digirep/index/SWORD_APP_Profile_1.2#Retrieve_Service_Document

still contains errors. The Example 2: level 1 has

<app:accept> elements, but the prefix "app" is nowhere defined and in
fact the default namespace is declared to be
"http://www.w3.org/2007/app", so no prefix is needed for the accept
element.
