There are a number of places in the book sources where extra whitespace is used to keep the source formatting sane, and some of those are having a direct result on the rendered results.

Some include:

* Index entries (extra spaces at the head of the paragraph):

    <para>
      <indexterm>
         <primary>...</primary>
      </indexterm>

      This is a paragraph...

* Screen output (extra newline before the output):

     <screen>
Some text
More text
Foo.
</screen>
