Version Control With Subversion Source

Resolve issue 1528.
In the book, cross-reference to "svn mkdir" from a point where it is likely to
be useful.
Based on a patch from John R. Daily <john_at_geekhavoc.com>.

* doc/book/book/ch04.xml (svn-ch-4-sect-6.1): Add xref to "svn mkdir".
* doc/book/book/ch09.xml (all command refentry tags): Add id attributes.
Remove some incorrect whitespace.

Apply the last of the non-controversial changes from Francois Beausoleil in
patches in issue 1528. Also fix a doc bug noticed whilst doing so.

description of import.
Convert short svn subcommands to full svn subcommands, as documented in
the book HACKING file.

* doc/book/outline.txt: Convert short svn subcommands to full svn subcommands,

Act on comments in issue 1568.

* doc/book/book/ch03.xml (svn-ch-3-sect-4.1): Rephrase the decription of
"merGed" to include the possibility that incoming repository changes were
the same as the local modifications to the working copy.

Fix a typo. Thanks to <rengmann@...> for noticing it.
