Book tweaks.

* book/ch01.xml (Quick Start): clarify that an imported directory is not a
working copy.

* book/ch03.xml (import): same.

Change the output filenames for the book so it is not ambiguously named.

Break lines at 72 columns and use the same <para> style as in the original
files.

- Don't use long lines anymore, but break at 72 columns. Use the same
