Change the output filenames for the book so it is not ambiguously named.

* doc/book/Makefile
book.html becomes svn-book.html
book.ps becomes svn-book.ps
book.fo becomes svn-book.fo
book.pdf becomes svn-book.pdf

* doc/book/book
Updating svn:ignore to ingore the new filenames.
